Conducts interview to all patients seeking admission/treatment to determine eligibility for MSS assistance.
Provides outright counseling with patients and relatives before discharge patient in the hospital.
Prepares referral letters and other communication necessary for networking, linkages and coordination with community resources.
Attends and facilitates patient’s medical needs as well as the complaints.
Regular recordings of progress notes/reports of abandoned/referred patients.
Performs other duties that may be assigned from time to Time.

Qualifications

Minimum Qualifications:

Salary: Php 25, 816.00

Status of employment: Contract of Service
