About the Role
SmartScale360 is seeking an Intake Coordinator / Home Care Client Liaison to support our U.S.-based home care clients. You’ll be the first point of contact for potential clients and families, managing inquiries, coordinating assessments, and ensuring smooth intake operations. This role combines care coordination, administrative excellence, and digital communication skills, including social media support to enhance agency visibility.
Key Responsibilities
- Intake & Client Coordination
- Serve as initial contact for clients via phone, email, or social media
- Conduct preliminary screening to determine service needs
- Schedule in-home or virtual assessments and coordinate follow-up care
- Maintain accurate client and caregiver records in WellSky, Access Care, or EMR platforms
- Ensure compliance with documentation requirements
Home Care Coordination Support
- Monitor caregiver availability and match clients with suitable caregivers
- Assist in schedule management, absences, and replacements
- Support quality assurance and follow up on client satisfaction
- Prepare daily and weekly intake and coordination reports
Social Media & Digital Communication
- Manage and update agency social media accounts (Facebook, Instagram, LinkedIn)
- Draft posts promoting home care awareness, caregiver appreciation, and community engagement
- Track post performance metrics and report suggestions
Administrative & Reporting
- Maintain HIPAA-compliant client records
- Support general administrative tasks, data entry, and reporting
Required Qualifications
- 1+ year experience in home care intake, client services, or care coordination
- English communication skills, written and verbal
- Proficiency in CRM or EMR tools (WellSky, Access Care, AlayaCare)
- Familiarity with home care regulations and intake workflows
- Strong attention to detail, organization, and multitasking
- Reliable remote work setup
- NOTE: It is ideal to to secure an NBI clearance upon application to avoid delays in matching

- Device: Intel i5 + 8 GB RAM minimum
- 100 Mbps internet

- Experience with U.S. healthcare accounts
